GOIVF Embryo Freezing in India

Embryo freezing, or cryopreservation, adds an important dimension to assisted reproduction by:
  • Extending the possibility for pregnancy when fresh cycles fail or when couples want additional children after a successful embryo transfer.

  • Avoiding many ethical dilemmas by eliminating the need to dispose of embryos.

  • Offering an alternative to couples that might transfer too many embryos and risk a multiple gestation pregnancy.

  • Avoiding embryo wastage by freezing embryos individually for efficient use.

  • Increasing pregnancies per retrieval cycle with normal outcomes by 10-30% more. Many studies have evaluated the children born from frozen embryos. The result has uniformly been positive with no increase in birth defects or development abnormalities.


We define embryo survival based on the number of viable cells in an embryo after thawing. An embryo has “survived” if >50% of the cells are viable. We consider an embryo to “partially survive” if < 50% of its cells are viable, and to be “atretic” if all the cells are dead at thaw. Approximately, 65-70% of embryos survive thaw, 10% partially survive, and 20-25% are atretic. Our data suggests that embryos with 100% cell survival are almost as good as embryos never frozen, but only about 30-35% survive in this fashion.

Embryo morphology (appearance of the cells / percentage of fragmentation) is one of the most influential factors for embryo survival. Interestingly, embryos produced from intracytoplasmic sperm injection (ICSI) also seem to survive somewhat better than embryos produced from conventional insemination. The following graph illustrates these points. The embryo grade in the graph goes from worst (3.2) to best (1.0).
Embryos that are 2, 4, or 8 cells when frozen have about 5-10% greater survival than embryos with an odd number of cells. Donor egg embryos have a 2-5% greater survival rate than embryos from infertile women when compared by morphology score.
Pregnancy rates are similarly affected by complex relationships and like embryo survival only 7-10% of the predictive value can be observed and measured. Age is not a significant factor with frozen embryos but fewer older women have frozen embryos. From the approximately 20 factors reviewed, the most important factors predicting pregnancy rates are the number of surviving embryos transferred, the number of 100% surviving embryos transferred, and the morphology scores of the transferred embryos. The delivered pregnancy rates ranged from 5% (a single poor quality embryo) to 36% (4 high quality embryos) when the cycles from 1987 to 2001 were combined.
Blastocysts (embryos cultured for 5 days rather than 2-3) are a special case. The embryos are much larger and have special needs with regard to freezing without damage. Many centers have had trouble with blastocyst cryo-survival and pregnancy rates. A new protocol developed in our laboratory and implemented in December 2000 led to a transfer rate of 62% and a 35% pregnancy rate per transfer. This important change now makes blastocyst transfer more appealing since excess blastocysts can be expected to yield pregnancy rates comparable to embryos frozen two to three days after retrieval.

What is Sperm Freezing?

Sperm freezing is used in artificial insemination, in vitro fertilization, surrogacy and other assisted reproductive procedures. Sperm freezing allows for a process of freezing and storing semen samples so you and your partner may use them to become pregnant in the future.

A Good Candidate for Sperm Freezing

Storing sperm prior to an IVF, egg donor or surrogacy cycle ensures the availability of sperm should the male partner have difficulty producing a sample on the day of egg retrieval or should the male not be available to produce a fresh sample due to other medical reasons.
Some men discover they have diseases or must undergo treatment or surgery that will cause permanent sterilization or genetic damage. As cancer detection and treatment techniques improve, more cancer conditions are being detected at younger ages in men, with better and longer survival rates. Therefore, more men in their fertile years are now considering this option.
Good candidates include:
  • In cases of low sperm counts where multiple semen collections may be necessary for use in a single inseminat To preserve semen for deferred inseminations when an intimate partner is temporarily absent

How is sperm frozen?

When the first specimen is received by the clinic in India, each sample is assessed and evaluated microscopically. Samples are combined with protectant solution to shield during freezing and thawing. The samples are placed in vials that are coded and carefully frozen in nitrogen vapor.
Once frozen, the vials are immersed in liquid nitrogen and stored in cryogenic tanks at a temperature of -196°C (-321°F). The liquid nitrogen is independent of any source of power. Cryogenic tanks are tested daily and refilled as needed.

How is sperm thawed?

When it is time for the sperm samples to be thawed, the cryopreserved sperm are checked of good quality. Once the sperm is thawed, it is placed into the female partner’s uterus in a process called intrauterine insemination (IUI), in vitro fertilization (IVF) or intracytoplasmic sperm injection (ICSI).

What types of surrogacy are available and what do they entail?

How soon can I plan to come now?

Can I choose the gender of my child?

No, gender selection is prohibited by law in India.
Can we choose to try for a multiple birth?

General step-by-step breakdown of an IVF treatment cycle in India

Stimulation of Egg Growth
Naturally a driving woman will ovulate only one egg each month. To increase the chance of obtaining pregnancy, an effort is made to recruit as many eggs as possible. This goal is achieved by fertility drugs (Goal-F(R)). The growth and development of the eggs is closely monitored by repeated ultrasound studies and blood tests for hormone levels (Estradiol and Progesterone). Based on the information obtained from these tests, the optimal timing for ovulation is determined. Ovulation itself is triggered by an injection (Human Chorionic Gonadotropin), after which the egg retrieval is scheduled.

Egg retrieval
Egg retrieval is scheduled some 36 hours after triggering ovulation using Injection HCG-Human Chorionic gonadotropin. The procedure itself is performed at the hospital by a Transvaginal route. A needle guided by ultrasound imaging, is inserted through the vaginal wall into the ovaries, where the follicles containing the eggs are punctured and aspirated. The released eggs are transferred to the lab where their developmental stage is assessed. General anesthesia is usually used for this simple and minimally invasive day care procedure. The procedure takes about 10-15 minutes, and the patient can return home a few hours after it.
The IVF /In Vitro Fertilization procedure
Sperm is obtained by masturbation. It then undergoes a series of lab procedures to prepare it for interaction with the egg. Fertilization Eggs are kept in dishes to which sperm is added at the proper concentration. The dishes are kept in an incubator where the environment (temperature, humidity, gas composition) is carefully monitored. In given time intervals the eggs are assessed for fertilization, and subsequent divisions. The fertilized egg divides into two daughter cells, which continue to divide rapidly as shown in the adjacent photographs. This assessment is based on morphology.
Hence the detailed chromosomal composition of the embryos cannot be addressed without further tests.
Embryo transfer
Embryos that have satisfactorily divided are transferred to the uterus 2-3 OR 5 days after fertilization. This simple and painless procedure does not require sedation or anesthesia. The embryos are loaded on a small plastic catheter, which is gently introduced through the cervix into the uterus. Once in the uterine cavity, the embryos are gently released and the catheter is withdrawn. The patient can return home after a couple of hours of rest.
Follow-up
Generally, we recommend that following embryo transfer the patient should refrain from physical exertion, however complete bed rest does is not needed and infact maybe counter affective. Just take it easy and hope for good news. Given the nature of cycle stimulation, we prescribe medications to support the young embryos hormonally. The presence of pregnancy is established by using a sensitive test for Human Chorionic Gonadotropin (HCG), which is the hormone secreted by the placenta. If pregnancy is established, monitoring is continued, until ultrasound imaging allows a direct visualization of the developing fetus and heart beat at day 28 following embryo transfer.
Advanced Artificial Reproductive Technology
Recent years have witnessed a rapid advancement in the treatment of the “male factor infertility”, that is, those couples where the husband cannot produce good quality sperm in the required concentration. Novel techniques have been developed with which a single sperm is injected into the egg. This procedure, referred to as ICSI (Intra Cytoplasmic Sperm Injection) can accomplish fertilization with only minimal number of viable sperm. Egg donation is the process by which a female donates her eggs for another female for purposes of assisted reproduction. The need for females to seek donated eggs may arise for any number of reasons including:

  3. Psychological screening provided by the agency
  4. An additional medical and social history performed by IVF clinic
  5. Physical examination performed by IVF specialist
  6. Infectious disease testing
Over and above these screenings, most patients who are going to use an egg donor will also look at many personal issues of importance such as:
  1. Physical appearance
  2. Ethnicity
  3. Interests and talents
  4. Reason for donating. There should be some indication that a donor has some interest in donating beyond simply the money. In our opinion when a donor has some altruistic motivation in addition to the money, they seem to be more compliant in terms of their treatment protocol.
One additional issue that we counsel patients to be aware of is that there is a difference between a “proven donor” and one that has never donated before. A “proven donor” is a donor who has donated before and whose donation has resulted in a pregnancy. Such donors obviously remove much of the guess work in an egg donor cycle. We have seen a higher pregnancy rate with “proven donors” as opposed to young ladies who have not donated before. Having said that however, this does not preclude patients from using donors who have not donated before if they fulfill other criteria that are of importance to the patient. It is just an additional element for patients who are looking at egg donation to be aware of in their decision making process.

Egg Donor IVF Treatment Process:

We seek your arrival schedule in India and the egg donor is given medication to stimulate her ovaries to grow multiple eggs accordingly. While her eggs are growing, the IVF specialist will prescribe the recipient of the donated eggs to take a few medications even prior to arrival in India. She will take hormones for about two weeks to synchronize her cycle with the donor’s cycle. Therefore the female intended parent’s uterine lining develops as the donor’s eggs grow which produces the implantation environment for the embryos which would be transferred two to five days after the egg retrieval.

Success rates with donor eggs:

Success with donor eggs is always higher as compared. The success rates of donor egg IVF / ICSI at Go IVF clinics in India is more than 60 %.

How many embryos will be transferred in me?

The decision of the number of embryos to be transferred is taken by the IVF specialist after the IVF Treatment procedure. As per the embryo quality, number of mature embryos, cell division etc. she will take a call on the number of embryos to be transferred in you. She will also consult you and then does the transfer. The IVF specialist will normally transfer 3 - 4 good looking embryos in the female intended parent.

Timeline and Stay in India

As you are coming for IVF / ICSI with an egg donor, the donor would be started on medications prior to your arrival and your stay would be for 10 days in India.
